Wireless Joint

It makes sense to set a PIN on the SIM to prevent use in case of theft. I tried unsuccessfully to set a PIN on a simplemobile SIM and it is now locked. I only now realize there is no PUK on the packaging. Upon login to their website, I'm told the card is no longer usable and I must call them.

Has anyone else run into this? Why would they not provide the PUK?
Does T-Mobile or other carriers provide the PUK?
