Hi,
your modem is detected properly as a network card.
So you can activate it with NetworkManager (run DHCP client on it). Or manually run dhclient NET_LINK_NAME
There is no need to use ModemManager (mmcli) in your case.
While it disappears - check logs. journalctl -r
