This one (photo attached) has a single QR code, which does not look like the IMEI. Others I've seen on the Net have 2 QR codes. The top one and another one below for the IMEI.

EM7511 under ROOter
I have a EM7511 with an AT&T SIM. The SIM's ICCID is bound to the IMEI of the card (found by querying the modem under Windows). The W10 Sierra drivers have been installed, W10 recognizes the card and the AT&T SIM, but can't get a connection.

I got a Sierra EM7511, which I installed in this HM2U V2 USB to M.2 adapter enclosure:
