I'm experiencing some issues with the newer USB 3.0 M.2 Modem sleds from LTEFIX.
I've purchased 2 of these, and a coworker has also purchased one, and all 3 are experiencing the same issue.

That I've not seen myself, the settings reversion. Might be a NVRAM problem? I don't think the Sierras act like the Quectels when it comes to that, I've never heard of a command like the =1 that matches Sierras RMARESET command, though I've never queried it via AT!RMARESET=?

Possibly a long shot but have you tried switching out the Sim during your troubleshooting search?
Switching out the SIM? At the moment I only have one Calyx SIM card, as I only have one account I use to test with, or I use it on the road.
Make sure you have "Drop Invalid Packets" enabled ...

Could be a tower based issue. Can you test the setup in a different tower zone to see if the disconnects still happen?

After you ran the deactivate command, did you reset the modem via at+cfun=1,1 ?
Its possible the EM12 has issues with the MBN profile commands, it never really works right on the EM12. Which firmware are you using again? Not sure if you said.

On the initial startup (which you define as a good start up) we see the default APN of "ims" set in profile slot 1.

A power outage may have corrupted the modem's NVM (Non-Volatile-Memory).
I would reset it and try both QMI and MBIM modes.
QMI mode
Copyy and paste each command one at a time, and make sure you get an "OK" response after each command:
